Ryhmäkuntoutusmalli muistisairautta sairastaville asiakkaille

Tämän opinnäytetyön tarkoituksena oli kehittää muistisairautta sairastavien asiakkaiden ryhmäkuntoutusta siten, että kotona asuminen onnistuisi pidempään. Tavoitteena oli luoda ryhmäkuntoutusmalli, joka koostettiin eri toimijoiden ryhmäkuntoutukselle antamien merkitysten avulla. Opinnäytetyö toteutettiin toimijalähtöisenä ja osallistavana tutkimuksellisena kehittämistoimintana konstruktivistista lähestymistapaa mukaillen.
Tietoa tuottavina toimijoina kehittämistyöryhmissä olivat muistisairautta sairastavat asiakkaat ja omaiset, heidän kanssaan työskentelevät ammattilaiset sekä esihenkilöt. Aineistoa analysoitiin teemoittamalla ja tiivistämällä. Teoriatiedon ja tiedontuottajilta saadun aineiston pohjalta kehitettiin muistisairautta sairastavien asiakkaiden ryhmäkuntoutusmalli. Kehittämistehtävänä oli tuottaa ryhmäkuntoutusmalli muistisairautta sairastavien asiakkaiden ryhmäkuntoutuksen järjestämisestä Turun kehyskunnissa. Kehyskuntia ovat Kaarina, Lieto, Naantali, Perusturvakuntayhtymä Akseli (Masku, Mynämäki, Nousiainen) sekä Raision ja Ruskon yhteistoiminta-alue.
Tämän opinnäytetyön perusteella on tärkeää, että muistikuntoutus toteutuisi omana palvelukokonaisuutenaan tulevaisuuden hyvinvointialueilla Suomessa. Ryhmäkuntoutusta koordinoisivat muistikuntoutuskoordinaattorit ja vaikuttavaa ryhmäkuntoutusta olisi järjestettävä kaikissa muistisairauden vaiheissa....

As the population ages, the need for services, carers and service housing increases. Multidisciplinary competence in service housing enables the provision of care for older people while maintaining their functionality and supporting the staff in their work. The aim of this thesis was to develop multidisciplinary competence in service housing for older people. The thesis investigated the professional competence of physiotherapists and occupational therapists and how this competence can support multidisciplinary work. The thesis was implemented as a qualitative, research-based development activity. The development methods used were a Webropol survey and a Teams workshop. The survey was sent to the physiotherapists and occupational therapists working in Attendo therapy services. The results of the survey were analysed using content analysis with a focus on thematic analysis. The results showed that the job description of a physiotherapist and occupational therapist is broad and there is a high degree of similarity. As a result of the thesis development activity, job descriptions for physiotherapists and occupational therapists were created, which can be used in supervisory work, in the recruitment of these professional groups and work organisation. The job descriptions were compiled in a guide as part of the residential services of Attendo Homes. Although the terminology used in the development work referred to service housing, the job descriptions can be used in the context of existing communal housing and 24-hour service housing. In the future, it would be good to introduce therapy competence to other staff working in the unit, as this could support the effectiveness of multidisciplinary cooperation....

This thesis was commissioned by Hoivapalvelut Onnenpisara, a private provider of social and health care services, provider which has been granted the Green Care quality label. The purpose of this thesis was to develop Green Care guidance competence in the rehabilitation of older people. The aim of the thesis was to establish a Green Care guidance competence model for Onnenpisara and to provide compact information about the nature of Green Care guidance competence. The theoretical part of the thesis discusses previous research on gerontological rehabilitation as well as the concept of Green Care and guidance related to it. The thesis was conducted as a research-based development work. The data collection methods of the study included the funnel approach and the wall technique involving the instructors of Onnenpisara. The research material consisted of previous research which formed the basis for an integrative literature review, as well as of information acquired from practical work.The main viewpoints of the theme were defined using content analysis. The evaluation was conducted using self-reflection and the feedback received from Onnenpisara staff. The results of the study showed that Green Care guidance competence of an instructor comprises, in addition to methodological skills, social, instructor-specific, professional and reflection skills. The outcome of the research-based development work was a Green Care guidance competence model to provide the instructors with tools for developing the guidance-based approach and practical guidance activities. In organizations, the model can be used, for example, to explore the needs for developing guidance competence....

Placing short-term prisoners in facilities outside the prison requires coordination between various actors, i.e. the person being placed, the prison, Community Sanctions Office, substance abuse services organized by the wellbeing services counties, substance abuse treatment facility, the Health Care Services for Prisoners (VTH) and healthcare services organized by the wellbeing services counties. The aim of this thesis was to provide information about the development needs of the placement process in substance abuse rehabilitation facilities outside the prison. Another aim was to improve the mutual knowledge and cooperation of the actors involved in the process in the North Karelia region. In addition to the existing theoretical knowledge, three different methods were used to collect data. A survey was administered to the actors of the placement process. Those placed in substance abuse rehabilitation facilities outside the prison were interviewed. A workshop was organized for the actors involved in the placement process using the GROW model. This study indicated that one of the key development areas in cooperation with various actors is access to information and its distribution. The motivation and genuine desire of the convict to be rehabilitated was seen as the most important factor in favour of placement. Concerns related to the security of the placement were raised by both prisoners and actors. The integration of the status of a rehabilitee placed in a substance abuse facility outside the prison with that of a prisoner raised concerns. The results revealed that a clearer understanding of cooperation between the actors was reached.
